When you live out of town, have a demanding job, or are raising young children, it can be difficult—if not impossible—to regularly check in on a loved one in a nursing home or assisted living community. That’s where our EYES-ON Visits come in.
This service is designed specifically for families who want the peace of mind of an in-person check-in without the need for ongoing care management. Our experienced care team serves as your eyes and ears on the ground, ensuring your loved one is safe, comfortable, and receiving appropriate care.
What’s Included:
- In-person visit to your loved one in their facility (nursing home or assisted living community)
- Observation and interaction to assess general well-being, hygiene, mood, environment, and visible concerns
- Photo update or live video chat with your loved one (as appropriate and permitted)
- Brief written status report sent to the designated family contact following each visit
- Coordination with staff as needed to raise immediate concerns or clarify care issues
Ideal For:
- Adult children or family caregivers living out of state
- Families needing occasional oversight without full care management
- Individuals wanting independent confirmation of how their loved one is doing
- Peace of mind during travel, illness, or personal caregiving breaks
Frequency & Cost:
- $90 per visit (1 hour average, plus travel and mileage)
- Choose bi-monthly or monthly visits
