We offer a variety of services for those needing assistance including arranging for transportation, meal delivery, participation in physician appointments, overseeing in-home care, improving optimal outcomes, etc.
The cost of our services varies depending on the specific service requested. We offer competitive pricing and are happy to provide a quote for any service. We generally begin with a retainer to get services started ($600).
Following the start of care retainer, we bill the responsible party at the end of each month.
We are happy to assist our veterans in enrolling at our local VA Clinic. We have experience working with the Veteran Service Officers and understand the need of gathering the required documentation needed for filing benefits. We are NOT employed by the Veterans Administration, but we can assist in the process of helping the veteran navigate the system.
Our care managers are skilled and have backgrounds that enable them to provide compassionate and knowledgeable services. We prioritize safety, respect, and kindness in all of our services.
We are not Medicaid specialists, though we are happy to refer people to those who are.
We are open Monday through Fridays. While we do not have office hours in the evening, weekends or holidays, we do recognize emergencies occur. We will do our best to have a care manager available for clients needing emergency room care in after hours, as well as being available to fax information to the Emergency Room and follow up care.
We are able to do consultations via phone or zoom. We offer packages priced for convenience in 30 minute increments up to 5 hours per month. Consultations must be scheduled.
We are private pay and not covered by insurance, unless a long term care policy designates that our services are a part of the benefits. However, Brevard Healthcare Navigation does subcontract with a 3rd Party Provider who bills Medicare for the new 2024 benefits of Community Healthcare Integration (CHI) and Principle Illness Navigation (PIN) services. Medicare recipients must qualify for this assistance through a physician referral - it is designed to assist those who have been determined as at-risk under Social Determinants of Health (SDOH). This is not a benefit for all Medicare recipients. We do NOT bill Medicare.
