What This Service Includes:
Our care managers act as your healthcare partner, offering support through a range of personalized services, such as:
- Home Status Visits – Scheduled weekly, twice-monthly, or monthly to monitor wellbeing and living conditions
- Physician Coordination – Communicating with your medical team to ensure up-to-date care plans
- Appointment Attendance – We attend medical appointments to help ask the right questions, take notes, and relay information clearly
- Hospital Advocacy – If hospitalization occurs, we can oversee care, communicate with the care team, and help plan for safe discharge
- Crisis Intervention & Planning – Proactive involvement to help reduce preventable hospitalizations and improve health outcomes
